RESUMO
Com base na dupla polaridade doente e não-doente do toxicodependente, os autores tentam analisar os objetivos da intervenção em diferentes contextos terapêuticos (hospitalização, pós-cura, acompanhamento psicoterapêutico de longa duração e terapia familiar).
Eles definem a complexidade da tarefa do terapeuta face aos paradoxos da situação clínica do toxicodependente, para o que necessitam de uma grande variedade de estratégias de acompanhamento. Estas diferentes modalidades de intervenção não fazem da abstinência um fim em si. Trata-se sobretudo, para o terapeuta, de favorecer a modificação do funcionamento psíquico do toxicodependente.
Utilizando dois exemplos clínicos, poderemos avaliar a rigidez dos mecanismos em estudo nestes indivíduos, tanto no plano intrapsíquico como ao nível das interações familiares.
O objetivo terapêutico essencial será estimular processos de separação e de autonomização, abrindo brechas no espaço mental do toxicodependente e da sua família.

RÉSUMÉ
Partant de la double polarité malade et non-malade du toxicomane, les auteurs tentent de cerner les objectifs de l'intervention dans différents contextes thérapeutiques (hospitalisation, post-cures, prise en charge psychothérapeutique au long cours et thérapie familiale).
Ils définissent la complexité de la tâche du thérapeute face aux paradoxes de la clinique du toxicomane, nécessitant une pluralité de stratégies d'accompagnement.
Ces différentes modalités d'intervention ne feront pas de l'abstinence une fin en soi. Il s'agira surtout pour le thérapeute de favoriser la modification du fonctionnement psychique du toxicomane.
A travers deux exemples cliniques, on pourra mesurer la rigidité des mécanismes à l'oeuvre chez ces sujets, au plan intrapsychique comme au niveau des interactions familiales.
L'objectif thérapeutique essentiel sera d'amorcer des processus de séparation et d'autonomisation, en ouvrant des brèches dans l'espace mental du toxicomane et de sa famille.

ABSTRACT
Based on the double polarity between illness and non-illness of the addicts, the authors try to analyse the aims of the intervention in different therapeutic contexts (hospitalisation, post-recovery, long term psychoterapeutic attendance and family therapy).
They define the complexity of the therapist's task through the paradoxes of the clinical situation of the addict, which needs a variety of attendance strategies.
These different ways of intervention will not make of the abstinence an aim in itself. The therapist will have to promote the change of the psychical acting of the addicts.
By means of two clinic examples, we will be able to evaluate the severity of the mechanisms under study in these persons, in the intrapsychic level as well as in the family interactions.
The essential therapeutic aim will be to stimulate procedures of separation and of autonomisation, opening fissures in the mental space of the addicts and of their families.
